BECOMING ESTABLISHED  PATIENTS
TO BECOME AN ESTABLISHED PATIENT WITH OUR PRACTICE YOU MUST SCHEDULE AND BE SEEN FOR  YOUR FIRST APPOINTMENT. MOST OF THE TIMES THE NEW APPOINTMENT WILL BE A PHYSICAL.  YOU SHOULD CHECK YOUR INSURANCE BENEFITS TO MAKE SURE THEY COVER PHYSICALS.  AT THIS TIME MOST INSURANCE COMPANIES ARE COVERING PREVENTIVE SERVICES SUCH AS PHYSICALS ONCE EVERY THREE YEARS, BUT TO BE SURE PLEASE CALL THE NUMBER ON YOUR INSURANCE CARD.  ALSO IF YOU HAVE HAD A PHYSICAL WITHIN THE LAST THREE YEARS PLEASE TELL US.

ITEMS TO BRING IN WITH YOU:
1.  ANY OF YOUR PAST MEDICAL HISTORY YOU MAY HAVE
2.  YOUR INSURANCE CARDS  --  AND AS A REMINDER PLEASE CHECK YOUR BENEFITS, MANY INSURANCE   COMPANIES HAVE CHANGED BENEFIT PLANS AND YOU MAY HAVE A LARGER COPAY, A DEDUCTIBLE, A HEALTH SAVINGS ACCOUNT.  
ANY COPAYS, CO-INSURANCES AND OR DEDUCTIBLES ARE YOUR RESPONSIBILITY ON THE DAY OF SERVICE.
3. A LIST OF YOUR PRESCRIPTION MEDICINES, OR YOU JUST MAY WANT TO BRING THEM IN A BAG.
4. A PICTURE ID, SUCH AS YOUR LICENSE.
5. PLEASE COME IN FIFTEEN MINUTES EARLY TO FILL OUT PAPERWORK.
       
  • SOME RULES OF OUR PRACTICE
              1.
      NARCOTIC PRESCRIPTIONS: WE DO NOT GIVE OUT NARCOTICS ON THE FIRST APPOINTMENT, IF YOU HAVE A GENIUNE NEED FOR NARCOTIC MEDICATION, THE PROVIDER WILL REVIEW YOUR MEDICAL INFORMATION, AND OTHER INFORMATION SUPPLIED TO US BY PHARMACIES AND THEN MAKE A DETERMINATION.
              2.  APPOINTMENT CANCELLATION: PLEASE CALL 24 HOURS IN  ADVANCE IF YOU NEED TO CANCEL AN APPOINTMENT.   PLEASE REMEMBER ANOTHER SICK PATIENT MAY BE TRYING TO GET SEEN BY OUR PROVIDERS.  MORE THEN ONE NO SHOW WITHIN A 12 MONTH CYCLE WILL ACQUIRE A $25.00 CHARGE. 
               3. REFERRALS:   IF YOU ARE SEEING ANOTHER SPECIALIST AND THINK YOU MAY NEED A REFERRAL FROM YOUR INSURANCE COMPANY PLEASE CALL YOUR INSURANCE COMPANY TO SEE IF IT IS NEEDED.  IF ONE IS NEEDED PLEASE CALL AND SPEAK TO TINA.  REFERRALS ARE NOT AUTOMATIC, WE HAVE TO SUBMIT THEM TO YOUR INSURANCE AND THE INSURANCE COMPANIES NEED TIME TO REVIEW ANY INFORMATION WE GIVE THEM.  
  •             4. PRESCRIPTIONS:  PLEASE ALLOW 48 HOURS TO REFILL PRESCRIPTIONS, IF THE MEDICATION NEEDS AN AUTHORIZATION FROM YOUR INSURANCE COMPANY, IT MAY NEED A WEEK.  PLEASE KEEP TRACK OF YOUR MEDICATIONS, TO ALLOW TIME FOR REFILLS BEFORE YOU RUN OUT.  
                5. BALANCE ON ACCOUNT - NO APPOINTMENTS WILL BE SCHEDULED IF WE HAVE SENT THE PATIENT  TWO STATEMENTS AND NO PAYMENT OR COMMUNICATION WITH OUR BILLING DEPARTMENT HAS BEEN RECEIVED.  AFTER THREE STATEMENTS WE WILL CONSIDER DISCHARGING THE PATIENT FROM OUR PRACTICE.
